Conference Registration

I hereby certify that the information contained on this conference registration form is accurate. I also certify that by pressing the "Register" button below I am authorizing Florida NELA to charge my credit card provided on this form for the total conference registration fee above. In addition I am affirming that I have read and agree to the Terms and Conditions of our Refund Policy (see following paragraph). Our Refund Policy states that all refunds must be requested before the start of the conference. Requests for a refund will be taken on a first come, first serve basis. If Florida NELA can replace you with an on-site registrant, you will be entitled to a full refund. If Florida NELA can replace you with an on-site registrant who is paying a reduced registration fee (i.e. one day registrant) you will only receive a partial refund. If Florida NELA cannot replace you with an on-site registrant, then no refund will be granted. If you fail to notify Florida NELA of your inability to attend the conference in advance, you will not be entitled to any potential refund. Thank you and we look forward to seeing you there!
